As the home owner you will have to contact a licensed plumber to provide you with a quote to connect your existing sewer drain to the new system provided, this quote should include decommission of your septic tank.

After you have applied you will be issued with paperwork including PIC Consent to Connect Number, Conditions and Property Sewer Plan. These documents will need to be passed on to the plumber of your choice in order for a drainage inspection to be booked and a Certificate of Compliance to be issued

Your plumber of choice will connect your house to the new system according to the VBA standards making sure all fixtures discharge to the system, that there is an overflow relief gully, there is adequate ventilation for the drain and any existing non-compliant drains are upgraded or given exemption by the VBA.

At the end of completion your plumber will provide you with a certificate of compliance and submit an upgraded drainage plan to Yarra Valley Water

Your plumber will also need to decommission your septic tank.  Once completed your plumber will give you a Compliance Certificate to certify that all plumbing work complies with the relevant plumbing standards.
